Please be aware that in addition to this form you must complete the council's personal disclosure form (making sure you disclose all convictions, cautions and traffic offences etc. that you have ever received, whether unspent or not).
This application forms requires:
- an Enhanced Disclosure Certificate issued by the DBS, provided through the Council’s nominated third party supplier.
- a Group 2 medical in accordance with the Council’s Policy.
- a tax check code, if you are applying to renew
- proof of right to work in the United Kingdom or the share code issued by the Home Office right to work checking service (see note two)
- a passport style photo
- payment of the fee for the application - please have your payment details ready
