Once we have confirmed your identify we will process your request in accordance with the guidance contained on our website. You have the option to scan the documents and upload them to this form or alternatively bring copies into the Civic Centre.
We will confirm in writing once your request has been processed or if a decision has been taken that your request can not be processed.
In this form, we ask for some personal information in order to fulfil your request. This information is held securely until your request has been completed following which it will then be deleted from our records. Any processing of your information is carried out in line with the requirements of the Data Protection Act 2018 and the enshrined General Data Protection Regulations (GDPR).
